During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was conducted on April 30, 2025. The inspection began at 2:00 p.m. and ended at 4:45 p.m. The inspector reviewed compliance in the areas listed above. There were 45 children present and 7 staff. The inspector reviewed 6 children?s records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff 1, was employed for one month before a TB test or screening was obtained.
Plan of Correction: TB tests and screenings will be obtained prior to employment.
Standard 8VAC20-780-180-C
Staff who drive a vehicle transporting children shall disclose any traffic violation that occurred prior to or during employment or assignment as a driver. Staff 4 and Staff 5 reportedly transport children in center vehicles; they have not completed a driver statement disclosing traffic violations.
Plan of Correction: Drivers will be asked to complete a written statement regarding traffic violations prior to driving.
Standard 8VAC20-780-510-B
Medication shall be given to a child only with written authorization from the parent. One
emergency medication for one child did not have written authorization from the parent.
Plan of Correction: Written authorizations will be obtained from parents before accepting medications.
Standard 8VAC20-780-70
The facility failed to have required documentation in each staff file. Two references as to character, reputation, and competence should be checked prior to employment. Staff 3 did not have documentation of ta second reference on file until 4 days after employment.
Plan of Correction: References will be documented prior to employment.
