Inspector Notes

An unannounced monitoring inspection was initiated on May 23, 2025 in response to a self-report received by the licensing office on May 13, 2025 relating to an injury sustained at the facility. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The following actions or threats thereof are forbidden: physical punishment or roughly handling a child. On May 9, 2025, Staff 1 grabbed Child 1 (age 5) by the hoodie, pulling hard enough to cause the child to fall down to the ground. Other staff present observed a red mark along the child's neck from the force of the shirt.
