An unannounced inspection was initiated on September 2, 2025 and concluded on December 2, 2025 in response to a complaint received by the licensing office on August 27, 2025 relating to allegations of an incomplete injury report from January 20, 2025. The inspector reviewed 1 child?s record on-site. This inspection included document review and interviews.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all required components. On January 20, 2025, a child sustained an injury. Staff sent a text and an electronic injury report to the parent. The report did not include: staff present and treatment; any future action to prevent recurrence of the injury; and staff and parent signatures or two staff signature.
Plan of Correction: Staff will complete all required components of the injury reports.
