Documentation of a physical examination must be submitted for each child within 30 days after the first day of attendance. Child 1, enrolled for over 3 months, did not have documentation of a physical on file.
Plan of Correction: Staff will request documentation of a physical for Child 1.

Repeat violation. Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff 1, employed for 9 months, did not have a current TB test or screening on file.
Plan of Correction: Staff will get a current TB test or screening.

Children?s hands shall be washed with soap and running water after toileting or diapering. Staff changed a child?s diaper in the toddler room and did not immediately take the child to the sink to wash hands with soap and water. The child touched toys and the table before washing hands.
Plan of Correction: Staff will be reminded to wash children's hands immediately after diapering.
