Inspector Notes

An unannounced non-mandated monitoring inspection was initiated on 06/12/2024 and completed on 07/22/2024 in response to a self-report received by the licensing office on 06/12/2024 relating to supervision. The inspector reviewed 2 children?s records and 2 staff records on-site 07/22/2024. This inspection included:

? document review,
? tour of the facility, and
? interviews

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to background checks and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/01/2024. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-A
Based on record review, the center failed to ensure that all staff obtain criminal history search results every five years for all employees.

Evidence:
1. The record of Staff 1 (DOH 07/29/2019) did not contain documentation of a repeat criminal history check. Staff 1 has been employed for more than 5 years.
2. The current criminal history check is dated 07/13/2019.
Plan of Correction: Completed Criminal History check was completed and submitted a new search on 07/30/2024.

REPEAT VIOLATION

Based on documentation and interviews, the center failed to ensure that staff were in actual sight and sound supervision of children under 10 years of age.

Evidence:
1. When leaving the playground, two children (age 2) were left outside.
2. Staff 1 reported that once inside she began counting children and realized two children were missing. Staff 1 stated she ran outside to find Child 1 (age 2) being brought in by a volunteer.
3. Staff 1 further reported that the volunteer was unaware of Child 2 (age 2), also outside and walking behind the volunteer.
Plan of Correction: On 06/13/2024 a team meeting was held to review policies and procedures for transporting the children to and from the playground. On 06/27/2024, a staff meeting was held and the policies and procedures were reviewed with staff.
