Inspector Notes

An unannounced non-mandated monitoring inspection was initiated on 07/22/2024 and completed on 07/22/2024 in response to a self-report received by the licensing office on 07/22/2024 relating to supervision. The inspector reviewed 1 child?s records and 2 staff records on-site on (date). This inspection included :

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. REPEAT VIOLATION

Based on interview, the center failed to ensure that children under 10 years of age shall be within actual sight and sound supervision of staff.

Evidence:
1. Staff 1 reported that during an evacuation drill on July 18, 2024, Child 1(age 5) did not follow the class outside to a safe area. Staff 1 stated that it was realized Child 1 was not with the class when a head count was conducted.
2. Staff 1 came inside to find Child 1 (age 5) back on cot trying to go back to sleep.
Plan of Correction: A staff meeting was held to review the Fire Drill on 07/18/2024. Emergency plan and fire drill procedures were reviewed. On 07/31/2024, a repeat fire drill was conducted with procedures in place.
