An unannounced, on-site focused inspection was initiated on 03/20/2025 and completed on 03/20/2025, as a part of Consent Agreement. The Department entered into a Consent Agreement with the facility on January 7, 2025.The on-site inspection began at 10:40 am and ended at 12:50 pm. The inspector reviewed compliance in the areas listed above.There were 20 children present and 8 staff. The inspector reviewed 2 staff records on-site.

This inspection included: document review, interviews, and observations.
