During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/14/2025. The on-site inspection began at 9:38am and ended at 10:10am. The inspector reviewed compliance in the areas listed above. There were 11 children present and three caregivers; the point total was 33. The inspector reviewed two children?s records and two caregiver/household members records on-site on 4/14/2025.

This inspection included document review, tour of the home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a licensed physician, the physician's designee, or an official of a local health department.

Child #1 had a physical examination and immunization record that was not signed by a licensed physician, the physician's designee, or an official of a local health department.
Plan of Correction: I will ask the parents for it today.
