Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/21/2025. The on-site inspection began at 9:45am and ended at 10:34am. The inspector reviewed compliance in the areas listed above. There were 10 children present and 3 caregivers; the point total was 30. The inspector reviewed 5 children?s records and 3 caregiver/household members records on-site on 8/21/2025.
This inspection included document review, tour of the home, interviews and observations.

The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There were no physical exam records available for Child #4, who had been in care for over 11 months.
Plan of Correction: I already called the parent and requested it during the inspection.
