Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/13/2024 and completed on 9/13/2024. The on-site inspection began at 10:00am and ended at 11:35am. The inspector reviewed compliance in the areas listed above. There were 36 children present and 9 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Caregiver #1 and Caregiver #2, who had been employed for two weeks, did not have a completed national criminal background check.
Plan of Correction: The center will obtain the results of the national criminal background check for the staff.

The center must request a search of the central registry prior to the employee's first day of employment.

Staff #2, who had been employed for two weeks, did not have a central registry search requested.
Plan of Correction: The center will request a search of the central registry for the staff and prior to employment of any future staff.

Centers must obtain a completed Out of State Sex Offender Registry Search prior to the employee's first day of employment.

Caregiver #1, caregiver #2. and caregiver #4, who had been employed for two weeks, did not have a completed Out of State Sex Offender Registry Search.
Plan of Correction: The center will obtain the results of the Out of State Sex Offender Registry Search for the staff.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.

There was no allergy list in three classrooms where children are present.
Plan of Correction: The center will place allergy lists in the classrooms.
