The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Ground covering was exposed underneath the mulch on the playground, causing a tripping hazard. Paint was chipping on one of the metal climbing structures on the playground. The plastic coating on the chains on the swings was peeling off.
Plan of Correction: The center will have the playground areas repaired.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #4, was missing the work phone number of one parent who has custody.
