Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including phone contacts, documents review, and interviews. Observations were made virtually in two classrooms, bathrooms, and the gym. The license, and emergency supplies were inspected. Children were observed working at desks working on a health project, and infants were observed sleeping and playing. Children were in the gym lining up to go back to the classroom.
A monitoring inspection was initiated on 01/11/2022 and concluded on 01/20/22. The director was contacted by phone and a virtual inspection was conducted. There were 65 children present, ranging in ages from infant to pre-k with16 staff supervising and or present including the director and assistant director and cook. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on documentation review and interview, the center failed to have a central registry within 30 days of employment.
Evidence:
1. The inspector reviewed five staff records, staff record # 5 did not have a central registry check completed: the hire date was 12/28/2021.
Plan of Correction: The form has been mailed but it was not within the required time frame. Director will make sure background checks are sent within one week of hire.
Standard 8VAC20-780-130-A
Based on record review, the center failed to have an immunization record in a child's file (file # 4).
Plan of Correction: The director said it had been handed in but it was not in the file. If it can't be found the parents will be asked for another copy.
Standard 8VAC20-780-140-A
Based on interview and documentation review, a child did not have a physical in the record within 30 days,
Evidence:
1. Five records were reviewed, record # 4 (date of enrollment, 5/16/21) did not have a physical on file.
Plan of Correction: One person will be designated to be in charge of the children's files. The director stated it had been handed in but it could not be found. Director will find it or ask parents for another copy.
