Inspector Notes
An unannounced, self-reported on-site monitoring inspection was initiated on 07/01/24 and concluded on 07/01/24. The on-site inspection began at 9:45am and ended at 10:46am. The inspector reviewed compliance in the areas listed above. This inspection included interviews, tour of the facility, and document review.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

2Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.
A self-report, interviews, and review of documents determined staff #1 and #2 did not ensure the care, protection, and guidance of Child #1, 5 years old, when he exited the playground unsupervised on 6/25/24 at approximately 4:13pm and was found standing alone on the sidewalk in front of the building by parent at approximately 4:15pm.
Staff #1 and #2 did not know the child had exited the playground until the child was returned to them by the assistant director.
A self-report, interviews, and review of documents determined staff #1 and #2 did not ensure the care, protection, and guidance of Child #1, 5 years old, when he exited the playground unsupervised on 6/25/24 at approximately 4:13pm and was found standing alone on the sidewalk in front of the building by parent at approximately 4:15pm.
Staff #1 and #2 did not know the child had exited the playground until the child was returned to them by the assistant director.
Plan of Correction: A final written coaching plan and retraining of active supervision was completed with both staff. We are also changing the type of latches on the gates.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.
A self-report, interviews, and review of documents determined staff #1 and #2 did not provide sight and sound supervision of child #1, 5 years old, on 6/25/24 for approximately 2 minutes when the child exited the playground unsupervised and walked to the front of the building. The child was found on the sidewalk by a parent who was entering the building.
