Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 7/26/24 and completed on 7/26/24. The on-site inspection began at 9:35am and ended at 12:10pm. The inspector reviewed compliance in the areas listed above. There were 88 children present and 14 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
The files of staff #1 and #2 did not contain a tuberculosis screening. Both staff have been hired and actively working with the children for over a month.
Plan of Correction: Staff will immediately obtain the screening. Staff screenings will be obtained prior to hire in the future as required.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.
The medication authorization for child A had expired 2 months ago.
The medication authorization for child B had expired 5 months ago.
The medication authorization for child C had expired 1 month ago.
These emergency medications for children A-C were in the active administration medicine area at the center during the time of inspection.
Plan of Correction: The parents were notified of this immediately after the inspection. New authorization forms will be obtained and renewed as required moving forward.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain verification of proof of birth.
The file for child #2 did not contain proof of birth verification. The child has been enrolled and in care for over 10 months.
Plan of Correction: Proof of birth will be documented immediately at time of enrollment.
