Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on October 27, 2021 and concluded on November 23, 2021. The inspection was conducted to investigate a self-reported incident that occurred on October 20, 2021. Staff interviews were conducted. A video recording and documentation was reviewed. Based upon the information gathered during the investigation, the self-reported incident regarding rough handling was determined to be valid.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff interview and video recording documentation, a staff member utilized the forbidden action of physical punishment to correct a child's behavior. Evidence: Staff A admitted to pulling child 1's hair to "demonstrate how it felt" after child 1 pulled another child's hair. A video recording documentation of the incident was reviewed that cooperated the admission.
