Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was conducted to investigate a complaint regarding preventing the spread of disease. The investigation was initiated on December 29, 2021 and concluded on January 19, 2022. Staff interviews were conducted and documentation reviewed. There were no violations were issued as a result of this inspection due to insufficient corroborating evidence to substantiate the allegations.
