Inspector Notes

An announced monitoring inspection was initiated on 8/10/22 and concluded on 9/19/22. The inspection was conducted in response to a self reported incident reported to the department on 7/26/22 by the center. The incident was related to a 4 year old child sustaining an injury on 7/22/22, due to a staff member using inappropriate disciplining techniques, and the incident not being reported to the parents. This inspection was conducted in conjunction with two other local agencies. 4 center staff were interviewed, 3 staff records, 1 child's record and the center's discipline policy was reviewed. Classroom observations were made during the visit. The evidence gathered during the investigation do not support the allegations of non-compliance with standards or law. Violations not directly related to the allegations have been cited, and are listed in the Violation Notice. For any questions regarding this inspection, please contact me at (571)-835-4718 or mahrukh.aziz@doe.virginia.gov.
