Inspector Notes
An announced monitoring inspection was initiated on 8/10/22 and concluded on 9/19/22. The inspection was conducted in response to a self reported incident reported to the department on 7/26/22 by the center. The incident was related to a 4 year old child sustaining an injury on 7/22/22, due to a staff member using inappropriate disciplining techniques, and the incident not being reported to the parents. This inspection was conducted in conjunction with two other local agencies. 4 center staff were interviewed, 3 staff records, 1 child's record and the center's discipline policy was reviewed. Classroom observations were made during the visit. The evidence gathered during the investigation do not support the allegations of non-compliance with standards or law. Violations not directly related to the allegations have been cited, and are listed in the Violation Notice. Violations
21. Staff # 1 stated that she was one of three assistants that was working in the Happy Face Classroom on 7/22/22. Staff # 1 also stated that since Staff # 2 left the center, she has been working in that classroom with two other assistants.
2. Staff # 3 confirmed via e-mail dated 09/07/2022 that, ?Staff # 2?s last day was April 29, 2022. We are currently in the interviewing process to find a new, qualified preschool teacher?.
1. The center?s discipline policy states that, ?Staff shall always try to re-direct behavior of a child to prevent negative behavior. Staff shall use positively worded directions to help guide the behavior of the children. If a child is frustrated and/ or upset, ask the child to express their feelings with words (If age appropriate). Time out shall be used only when extremely aggressive behavior is being demonstrated by the child and the above acceptable methods of discipline were ineffective or bringing harm to another child?.
2. Staff # 1 stated that when handling children that are misbehaving, and when using their behavior management technique, they give ?a verbal warning the first time, a second verbal warning, and the third time you use a chair and put time out for 3 minutes?.
