Inspector Notes

An unannounced monitoring inspection was conducted on 11/7/22, from 10:00 am to 11:00 am. There were 20 children in direct care and supervision of 10 staff members. The children were observed engaged in circle time, learning about letters and colors, using learning worksheets and playing outdoors. There were no children on site with medications. 3 staff records, 3 children's records, emergency drills records and the injury report log was reviewed.

Based on records review, the center did not obtain documentation of immunization for each child before the child can attend the center.
Evidence:
1. Child # 1's first day of attendance was 10/24/22.
2. Immunization records Child # 1 have not been obtained to this date.
Plan of Correction: Centers will ensure immunizations are received prior to start date.
Standard 8VAC20-780-160-A
Based on records review, the center did not obtain a negative Tuberculosis (TB) screening for a new employee within 30 days of the day of employment.
Evidence:
1. Staff # 1's date of employment was 8/15/22.
2. Staff # 1's TB screening was dated 9/15/22.
Plan of Correction: Center will ensure TB test are performed at least 30 days prior to start date.
