Inspector Notes

This renewal inspection was initiated by licensing staff and concluded on 05/02/2024, between the hours of 10 am to 12:30 pm. There were 13 preschool children present, with 4 staff supervising, across 2 classrooms. The children were engaged in the following: outside time, bathroom breaks, reading, learning, hand washing, story time, music, song, lunch, getting ready for naptime, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication (no meds were on site); special care and emergencies; and nutrition. A total of 5 children files, and 5 staff files were reviewed.

Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the center.

Based on review, 1 of 5 children files indicated that each child did not have a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance.

Evidence -
1. On the date of inspection (05/02/2024), documentation of a physical examination was not available for review for Child #3 (start date - 08/14/2023).
2. It was noted within the record (included with the immunization page) that a physical examination was conducted on 05/18/2023 for Child #3, but no further documentation of the physical itself was provided.
Plan of Correction: We will request the documentation needed from the parent and place in the child file.
