Inspector Notes

The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the department on March 21, 2024, regarding allegations of staff training and physical plant. The inspection was initiated and completed on April 2, 2024. The inspector was on site from 9:45am to 12:15pm. Physical plant was inspected, staff records were reviewed for required trainings.
The information gathered during this investigation does not support the allegations, so the complaint is determined to be not valid. There are no violations resulting from this complaint investigation.
