Inspector Notes
An unannounced monitoring inspection was conducted today, September 14, 2023, to determine the centers compliance with licensing standards. The inspector was on site from approximately 2:30 pm until 4:00 pm. The interim site Director arrived shortly after the inspector?s arrival and assisted with the inspection. Upon the inspector?s arrival there were 20 children in care. Children were playing table games and coloring. An additional 15 children arrived from another elementary school for a total of 35 children with two staff supervising. Children ranged in age from five years to eleven years.
Later children had a snack and played outside. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Staff interacted with children in a positive manner. A total of five child records were reviewed. Three staff records were reviewed virtually on September 18, 2023. The inspector interacted with staff and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Evidence:
1. The national fingerprint check for Staff #3, hired on 02/05/2023, was dated 02/07/2023 exceeding the first day of employment.
2. An administrator acknowledged the fingerprints background check was not completed prior to employment.
Evidence:
The record of staff #3, employed 02/05/2023, contained a sworn statement dated 02/16/2023 exceeding the date of employment.
Evidence:
The record of staff #3, employed 02/05/2023, contained documentation of a tuberculosis screening dated 02/07/2023 exceeding the first day of employment.
Evidence:
1. The record of staff #3, employed 02/05/2023, did not contain documentation of the completion of the Virginia Department of Education-sponsored orientation, exceeding 90 days from the date of employment.
2. An administrator stated staff #3 had not completed the Virginia Department of Education-sponsored orientation training.
Evidence:
The record of child #4, with a diagnosed food allergy, did not have documentation of an allergy care plan.
The director stated an allergy care plan had not been obtained.
