Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 5/24/21 and concluded on 5/26/21. The director was contacted by telephone to initiate the inspection. There were 11 children present and 2 staff present. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

2. The record for child #2 did not contain the address for both emergency contacts, and did not indicate whether or not the child had previously attended child care.
3. The record for child #3 did not contain a complete address for both emergency contacts, and did not indicate whether or not the child had previously attended child care.
4. Staff #4 (Program Administrator) confirmed that the records for child #1, child #2, and child #3 did not contain all of the required information.
