Inspector Notes

An unannounced inspection was conducted on October 17, 2022 from 7:10am to approximately 9am. There were 14 children present and three staff members. A sample size of 8 records were reviewed which included five children?s records and three staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during free play, coloring, and morning snack. First aid kits were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site director at the exit interview.

Based on observation and interview, it was determined that the center did not ensure that a separate record for each child enrolled which shall contain the work phone number of each parent who has custody.
Evidence:
1. Child #1 (date of enrollment 9/06/22) did not have their mother?s work phone number in their record.
2. Child #2 (date of enrollment 9/06/22) did not have their mother?s work phone number in their record.
3. Child #3 (date of enrollment 9/06/22) did not have their father?s work phone number in their record.
4. Staff #1 confirmed that Child #1, Child #2, and Child #2 did not have the work number of their custodial parent.
Plan of Correction: The center responded with the following: Staff #1 will obtain the work phone numbers for Child #1, Child #2, and Child #3 at pick up this afternoon.

Based on observation and interview, it was determined the center did not ensure that the findings of the most recent inspection of the center were posted on the premises.
Evidence:
1. The results from the most recently inspection was not posted in the center.
2. Staff #1 confirmed that the most recent inspection results was not posted in the center.
Plan of Correction: The center responded with the following: Staff #1 will post the acknowledge form from today?s inspection and post all future inspections in the center.
