Inspector Notes
An unannounced, on-site monitoring inspection was initiated on October 17, 2025 and completed on October 30, 2025. The on-site inspection began at 4:42pm and ended at 6:00pm on March 17, 2025 and began at 3:35pm and ended at 5:00 pm on March 30, 2025 in response to a self-report received by the licensing office on October 10, 2025 relating to procedures for emergencies. The inspector reviewed compliance in the areas listed above. There were 10 children present and 2 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to ongoing training and are documented on the violation notice issued to the program.
