Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 2/13/2025. The on-site inspection began at 4:40pm and ended at 5:45pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 4 children present and 2 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the name, address and phone number of two designated people to call in an emergency if the parent cannot be reached. The records for Child #2 and Child #4 did not contain the address of an emergency contact.
