During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/11/25. The on-site inspection began at 4:00pm and ended at 5:45pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 19 children present and 2 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-240-B
Staff shall complete orientation training appropriate to the ages of the children in care and include all of the facility specific topics, prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. Staff #1,who began working in the facility on 9/2/25, did not have documentation of completed orientation training.
Plan of Correction: The facility responded: Documentation of orientation will be completed and placed in the staff member record.

Repeat Violation
There shall be at least two staff members with certification in cardiopulmonary resuscitation (CPR) and First Aid present on the premises during the center's hours of operation, on fieldtrips, and wherever children are in care. There was only one staff member present with CPR/First Aid certification.
Plan of Correction: The facility responded: Staff # 1 will complete CPR/First Aid training on 9/12/25.

Repeat Violation
Staff records shall be kept for each staff person that includes all of the required information. The record for staff #1 did not include the date of employment.
