Inspector Notes

An unannounced monitoring visit and facility tour was conducted on 02/15/2023. At the time of inspectors' arrival there were 18 school aged children in care with 2 staff members. Children were observed interacting with staff, engaged in outdoor play, and departure. A sample of 5 children's records and 2 staff records were reviewed. Site Director reported that there have been no injuries and medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review and interview it was determined the center did not ensure that there was a record at the center that contained all the required documentation for each child enrolled.
Evidence:
1. The records for child #1, 2, and 3 did not contain the addresses for the two required emergency contacts.
2. The record for child #1 did not contain verification of identity.
3. The record for child #5 did not contain parent home and work contact information, emergency contacts, persons authorized to pick up, and program(s) previously and/or concurrently attending.
4. Staff confirmed this information was not available during this inspection.
Plan of Correction: Director will request documentation from parents.

Based on record review it was determined the center did not have the required signed parental agreements between the parent and the center in each child's record by the first day of the child's attendance.
Evidence:
1. The record for child #5 did not contain the required signed parental agreements.
