Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A monitoring inspection was initiated on 11/16/2021 and concluded on 11/18/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 21 children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center did not obtain an out of state sex offender name search or out of state criminal history background check prior to employment for all staff who have lived outside the state of Virginia within the last 5 years.

Evidence: Staff #1 (Date of Hire: 9/21/2021) has lived in the state of South Carolina within the last 5 years. The South Carolina sex offender name search and the South Carolina criminal history background check for Staff #1 were both completed on 11/18/2021.
