Inspector Notes

This inspection was conducted by staff using an alternative remote protocol, including phone interview and records review. An on-site inspection was not conducted at this time.

An unannounced subsidy inspection was conducted at 9:45am. The vendor was contacted by phone to initiate the inspection. There were no children present during the phone calls during this time period. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Based on records reviewed, it was determined that the vendor did not complete a required annual training. Evidence: There was no documentation that the Virginia Health + Safety Training class was completed in the last year.
Plan of Correction: The vendor will complete the training in the next 30-days.
