An on-site inspection was conduced on October 26, 2022. An unannounced subsidy inspection was contacted approximately 10:25 am. There were no children present during the inspection. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed.

Based on child records review, the vendor did not have documentation to ensure compliance with any of its own policies that have been disclosed to the parents of an enrolled child.
Evidence:
1. Child #1 did not have information to parents form completed and signed by parent.
2. Child #2 did not have sick policy, emergency medical authorization, child record form, information to parents form.
