Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A monitoring inspection was initiated on 08/04/2021 and concluded on 08/11/2021. The program director was contacted by telephone and a virtual inspection was conducted. There were 40 children, ages 6 months-5 years, and 10 staff present during the inspection. The inspector reviewed compliance in the areas of administration, staffing and supervision, emergencies and nutrition. A total of 3 children?s records and 3 staff records were reviewed during the inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and discussion with staff, the center failed to ensure that an employee must not be employed until the agency has the persons completed sworn statement or affirmation.

Evidence: The record for Staff #2 had a documented hire date of 07/06/2021. There was not a sworn statement or affirmation available for review in the record for Staff #2.
Plan of Correction: Will be completed today.

Based on record review and discussion with staff, the center failed to obtain a copy of the results of sex offender registry check from any state in which the individual has resided in the preceding five years.

Evidence: The record for Staff #1 has a documented hire date of 07/06/2021. The record for Staff #1 contains documentation of having resided outside of the state of Virginia within the preceding five years. There was not a check of the sex offender registry for that state available for review in the record for Staff #1.
Plan of Correction: All out-of-state checks will be done before hiring.
