Inspector Notes
An unannounced monitoring inspection, resulting from a self-reported incident, was conducted on-site on 05/10/2023 and concluded with telephone interviews on 05/16/2023. The inspector reviewed 1 child record and interviewed 4 staff. The inspector arrived at the center at 9:50 am and departed at 10:45 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the center failed to ensure that the separate record for each enrolled child shall contain all of the elements as required by the standards.
Evidence: The record for Child 1 did not contain the following information: name and phone number of the child?s physician; and the first date of attendance.
