Inspector Notes

An unannounced renewal inspection was conducted on 01/09/2024 and additional information was reviewed electronically on 01/16/2024. There were 29 children, ages 4 months-5 years, and 10 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. A total of 5 children?s records, 5 staff records, 1 applicant record, and 1 medication were reviewed. The children were observed participating in music and movement, free choice center play, during lunch and going down for rest time; infants were observed being fed and sleeping on demand. The inspector arrived for the inspection at 9:45 am and departed at 12:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-770-60-C-2
Based on record review, the center failed to obtain the findings from a search of the central registry within 30 days of employment.

Evidence:
1. The record for Staff 3 contained a documented hire date of 08/29/2023. There were no findings from a search of the central registry for Staff 3 available for review during the 01/09/2024 inspection. Staff 3 was observed working during the inspection.
2. The record for Staff 4 contained a documented hire date of 11/20/2023. There were no findings from a search of the central registry for Staff 4 available for review during the 01/09/2024 inspection. Staff 4 was observed working during the inspection. The results have since been obtained and are dated 01/17/2024.
3. The record for Staff 5 contained a documented hire date of 10/26/2023. There were no findings from a search of the central registry for Staff 5 available for review during the 01/09/2024 inspection. Staff 5 was observed working during the inspection. The results have since been obtained and are dated 01/17/2024.
Plan of Correction: Starting in 2024 all important dates will be kept on a calendar and will be reviewed regularly.
Standard 8VAC20-780-240-A
Based on record review and staff interview, the center failed to ensure that staff shall complete the Virginia Department of Education sponsored orientation course within 90 calendar days of employment.

Evidence:
1. The record for Staff 1 contained a documented hire date of 01/09/2023; there was not documentation that Staff 1 has completed the VDOE orientation course in the record for Staff 1. Staff 6 verified that the training has not been completed.
2. The record for Staff 2 contained a documented hire date of 08/29/2023; there was not documentation that Staff 2 has completed the VDOE orientation course in the record for Staff 2. Staff 6 verified that the training has not been completed.
Plan of Correction: Staff have been instructed to complete training or employment will be suspended until completed.
Standard 8VAC20-780-245-A
Based on record review and staff interview, the center failed to ensure that staff shall complete annually a minimum of 16 hour of training appropriate to the age of children in care.

Evidence: The record for Staff 1 did not contain documentation that a minimum of 16 hours of annual training had been completed. Staff 6 verified that Staff 1 has not completed 16 hours of training.
Plan of Correction: Reminded all staff to complete 16 hours of training annually.
Standard 8VAC20-780-260-A
Based on document review, the center failed to obtain an annual fire inspection report from the appropriate fire official having jurisdiction.

Evidence: The most recent fire inspection report was dated 06/01/2022.
Plan of Correction: Will call and request an inspection.
