Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 01/10/2025. The on-site inspection began at 9:50 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 33 children and 11 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-510-E
The center?s procedures for administering nonprescription medication shall be consistent with the manufacturer?s instructions for age, duration, and dosage.

Child 3 (10 months old) was administered Medication 1 five times between 10/25/2024-11/06/2024. The manufacturer?s instructions for this medication required physician?s instructions for administration to children under the age of 2 years; there were no physician's instructions available.
Plan of Correction: Will not accept medications without the doctors instructions if manufacturer's instructions do not allow without doctors instructions.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child as it occurs.

There were 7 children observed in the Infant Room but the attendance record had 8 children marked in attendance.
Plan of Correction: Will retrain the staff on the procedures to document when children are transitioning to or visiting a new class.
