Inspector Notes

An unannounced monitoring inspection was initiated on 09/16/2025 and completed on 10/07/2025 in response to a self-report received by the licensing office on 09/15/2025 relating to special care provisions and emergencies. The inspector reviewed 3 children?s records and 2 staff records. The inspection included document review, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to special care provisions and emergencies and are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-F
Children under 10 year of age always shall be within actual sight and sound supervision of staff.

09/15/2025, Child 1 (age 19 months) and Child 2 (age 15 months) were in the corner behind the diapering station at the kitchen door, while behind the diapering station, Child 2 smacked Child 1 in the head multiple times, the child began to cry. Staff 1 stated that Child 2 was outside of their sight supervision at that time.
Plan of Correction: Program Director went over classroom positioning with staff and it will reviewed again at the upcoming staff meeting.
Standard 8VAC20-780-410-1
The following actions or threat thereof are forbidden: roughly handling a child.

On 09/15/2025, Staff 1 roughly handled two children (age 15 months and 19 months) on three occasions. The 19-month-old child was later taken for outside medical treatment for an injury to their left arm.
Plan of Correction: The staff member's employment was terminated. Reviewed and retrained staff.
Standard 8VAC20-780-500-A
Children?s hands shall be washed with soap and running water after toileting (diapering) and staff shall wash their hands with soap and running water before and after a diaper change.

09/15/2025, Staff 1 changed multiple diapers and did not wash their hands before or after the diaper changes. Children did not wash their hands after the diaper change.
Plan of Correction: Retrained staff by each class.

The diapering surface shall be cleaned with soap and water and sanitized after each use.

Staff 1 did not clean the diapering surface with soap and water following a diaper change.
