Inspector Notes

An on site renewal inspection was initiated on 3/15/2022 and additional documentation of received on 3/17/2022. There were 78 children present, ranging in ages from 5 years to 12 years, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 new staff records were reviewed.
Information gathered during the inspection determined non-compliance with an applicable standard and violations were documented on the violation notice issued to the program.

Based on review of two staff files, the facility failed to have documentation of orientation by the end of the first day of employment. Evidence: Staff A (date of hire 11/16/2021) had documentation of orientation on 12/31/2021.
Plan of Correction: Staff A had orientation on 11/16/2021, but the documentation was misplaced. The facility gave orientation training again on 12/31/2021.
