Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on 2/23/22. There were ten children present with the provider and an assistant supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance with one applicable standard/law and the violation is documented on the violation notice issued to the program. Based on review of documentation, and an interview with the provider, it was determined that not all lockdown drills were practiced as required. Evidence: There is no documentation that an annual lockdown drill was conducted since the provider was approved as a subsidy vendor on 8/24/2020.
Plan of Correction: Provider stated she will conduct a lockdown drill annually.
