Inspector Notes

An unannounced monitoring inspection was conducted with the provider on August 14, 2023 beginning at 2:31pm and ending at 3:37pm. There were eight children, ages 12months, 13 months, 3, 3, 3, 3, 3, and 3 years old, in the care of the provider fand assistant or a total of 20points. The children were observed playing with age appropriate toys. Four children's records were reviewed. The provider's record and two household members' records were reviewed and in compliance. The provider has current certification in CPR and First Aid. The provider does not give medication at this time and is aware that Medication Administration Training (MAT) is required to do so. The first aid kit, flashlight, and battery operated radio were observed. The most recent emergency escape drill was conducted 8/2/2023. The most recent shelter-in-place drill was conducted 8/2/2023.

Based on a review of documentation, the provider did not obtain the parent's signed acknowledgment of the an annual review emergency contact information. Evidence: Child B's record did not have documentation acknowledging an annual review of the emergency contact information.
Plan of Correction: The provider will review the emergency contact information with the parent and obtain a signature acknowledging the review.
Standard 8VAC20-800-90-A
Based on a review of documentation, the provider did not obtain documentation of adequate immunization according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations prior to attending the family day home. Evidence: Child A's record was missing documentation of immunizations.
Plan of Correction: The provider requested the missing documentation. A copy of the documentation was obtained by the provider on August 17, 2023.
