Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child?s attendance or within 30 days after the first day of attendance.

Child A (enrolled 42 days) did not have documentation of a physical on file.
Plan of Correction: Child has returned all proper forms.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening.

Caregiver A did not have a current TB screening. The subsequent screening was overdue by 3 days.
Plan of Correction: TB Screening was completed next day.
Standard 8VAC20-800-210-A
Caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Caregiver B had a total of 8 out of the 16 required training hours documented for 2025.

Cribs shall be provided for children from birth through 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot, rest mat, or bed.

There were two infants observed in care (Ages 2 months and 3 months) there was only one crib observed. Provider stated one infant sleeps in a pack-in-play requested by parents.
Plan of Correction: Took second crib out of attic and setup that evening.
