Inspector Notes
An unannounced monitoring inspection was conducted on 9/18/2023 from 11:47am to 1:30pm. There were 25 (5 months to 4yrs) directly supervised by 7 staff. The physical plant, 2 staff records, 5 children?s records, no children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, eating lunch.. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
8Evidence: observed that the ceiling in the downstairs classroom had water damage from a water leak that created a large mold stain.
Evidence:
1) observed an unlocked hall closet with bleach, lysol spray, and cleaning agents accessible to children.
2) observed hand sanitizer on a child's bookshelf accessible to children in the toddler classroom.
3) observed in the 3s classroom: hand sanitizer on a bookshelf accessible to children and lysol spray unlocked in the bathroom.
Evidence: observed in the 3-4 yr old classroom, sunscreen that was not labeled.
Evidence:
1) observed in the infant classroom a small box of sample sized diaper ointments that were not labeled with children's names, or listed as facility provided ointments with parental approval.
2) observed in a child's supply box, diaper ointment that was not labeled with their name.
Evidence: reviewed emergency drill log and found that there was only 1 shelter in place drill conducted within the past 12 months.
Evidence: reviewed the emergency drill log and found that the facility did not practice a lockdown drill within the past 12 months.
Evidence:
1) children's food in the infant fridge that was not labeled with a name or a date.
2) observed a child's lunch brought from home was not dated.
Evidence: observed several bottles in the infant refrigerator were not labeled with children's names or dates.
