Inspector Notes

An unannounced monitoring inspection was conducted on 9/18/2023 from 11:47am to 1:30pm. There were 25 (5 months to 4yrs) directly supervised by 7 staff. The physical plant, 2 staff records, 5 children?s records, no children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, eating lunch.. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean. safe and operable condition.
Evidence: observed that the ceiling in the downstairs classroom had water damage from a water leak that created a large mold stain.

Based on observation, the facility did not ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1) observed an unlocked hall closet with bleach, lysol spray, and cleaning agents accessible to children.
2) observed hand sanitizer on a child's bookshelf accessible to children in the toddler classroom.
3) observed in the 3s classroom: hand sanitizer on a bookshelf accessible to children and lysol spray unlocked in the bathroom.
