During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 6/18/.25. The on-site inspection began at 10 am and ended at 11:55 am. The inspector reviewed compliance in the areas listed above. There were 30 children present and 7 staff. The inspector reviewed 6 children?s records and 4 staff records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
5The staff-to-children ratio of 1 staff to 5 toddlers ( 16 to 23 month old) must be maintained whenever children are in care.
One two occasions, for approximately one minute in duration per occurrence, there was one staff with 7 toddlers, the youngest child present was 18 months of age.
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.
A current allergy list was not kept in the Butterfly ( 3/4 year) classroom. Their list was dated 2/7/25 however the current allergy list was dated 5/29/25.
There was 6 inches of space between the service side of an occupied crib and a table.
A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
There was no allergy care plan for child A, who has a diagnosed allergy to dairy.
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
There was no attendance being maintained in the infant room, there were 5 infants present.
The attendance sheet in the 4/5 year old room had 12 children marked as being present when there was only 10 children present.
