Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.
The climber and slide on the older playground required a 6 foot fall zone, clear of all obstacles. The post to the awning was within 2 feet of the climber and slide.

Plan of Correction: We reminded the teachers to move and supervise children before and after playground time to ensure that a 6 foot distance is maintained between each piece of equipment.

There was no attendance being maintained in the infant room. There were 4 infants present.
