During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 3/11/26. The on-site inspection began at 11:13 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were 22 children present and 7 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat Violation:
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 and use zones for equipment. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.

1. On the large playground, the green slide was within 4 feet of the awning post.
2. On the rear playground, a climber/slide had no fall zone as it was located up against a fence.
3. The standard requires at least a 6 foot clearance/fall zone on all sides of play equipment with a climbing apparatus or equipment with moving parts.
Plan of Correction: We will remove the play equipment and in the future we will make sure we have adequate fall zones.
