An unannounced monitoring inspection was initiated on 12/10/2024 and completed on 12/20/2024 in response to a self-report received by the licensing office on 11/21/ relating to Supervision. The inspector reviewed children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, and interviews.
