22.1 BACKGROUND CHECKS, CODE, CARBON MONOXIDE
During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 1/8/2025 and completed on 1/27/2025 response to a complaint received by the licensing office on 12/22/2024 relating to allegations of programs, special care provisions and supervision. The inspector reviewed no children?s records and 1 staff records on-site on 1/8/2025. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
After review by the Licensing Administrator, the inspection documents were updated on 11/3/2025.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/03/2025. A POC submitted after this date will not appear on the public website.
Violations
5Staff #1, who had been employed for four months, had a national criminal background check completed 6 days after first day of employment.
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children.
The screening for Staff #1 was completed 3 days after hire.
Staff 2 was observed in the bathroom changing a diaper. When the staff was done, she removed her gloves and assisted the next child with changing soiled clothing. No handwashing was observed before helping the second child.
A nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designed for changing.
In the mixed ages classroom, teachers were observed changing the diaper of a child under the age of three. Staff stated they do not use a changing table or designed area for changing.
Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.
Staff #1, who had been working at the center for 4 months, had references on file that were 4 days after first day of employment.
