During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous
inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If
there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on 3/20/2025 and completed on 4/2/2025 in response to a self-report received by the licensing office on 3/3/2025 relating to supervision. The inspector reviewed 1 child?s record and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to backgrounds, tb screening and first aid/CPR.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/9/2025. A POC submitted after this date will not appear on the public website.
Violations
6Providers must obtain a completed national criminal background check prior to the employee's first day of employment.
Staff #2 had an eligibility letter dated 9 days after their first day of employment.
Staff #3, employed for seven months, did not have a completed national criminal background check.
The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.
Staff #2, employed for six months, had no documentation of a TB screening.
The TB screening for Staff #3 was completed 2 days after hire.
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.
Staff #2, employed for six months, has not yet completed the required course.
Children under 10 years of age shall always be within actual sight and sound supervision of staff.
On 2/28/2025, Child 1 (age 2 years) was left alone and unsupervised in a classroom for up to approximately 2 minutes. The center was previously cited on 8/29/2024 and 10/21/2024 for leaving a child alone and unsupervised.
Centers shall be in compliance with the center's own policies and procedures required by these standards.
The center?s supervision policy is that children shall remain under supervision of staff at all times.
On 2/28/2025, Child 1 (age 2 years) was left unattended in the classroom for approximately up to 2 minutes. The center was previously cited on 8/29/2024 and 10/21/2024 for leaving a child alone and unsupervised.
Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.
Staff #2 had reference checks completed 7 days after their employment.
