During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 8/22/2025 and completed on 9/5/2025, as a part of the probation period. The Department issued a Special Order on 5/27/2025 to place the licensee on probation for a period of 6 months and mandate training for all staff and any staff that supervise children. The on-site inspection began at 9:00am and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were 98 children present and 25 staff. The inspector reviewed 5 children?s records and 4 staff records on-site and electronically on 9/2/25.
This violation notice was amended on 9/11/2025.
After review by the Licensing Administrator, the inspection documents were updated on 10/31/2025.
This inspection included document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/11/2025. A POC submitted after this date will not appear on the public website.
Violations
10Staff #3, who had been employed for nine days, did not have a central registry search requested until 16 days after employment.
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child #1, who had been attending for over a year, did not have documentation of an immunization record on file.
Child #2, who had been attending for three months, did not have documentation of an immunization record on file.
Child #4, who had been attending for over a year, had immunizations on file that were 7 months past required timeframe.
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
There was no physical examination in the record of child #1, who had been enrolled for over a year.
The physical examination in the record of child #4 was 6 months past required timeframes.
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
The thermostats boxes in three of the classrooms were exposed with the cover off. These boxes were within the reach of the children in the classrooms.
In the pre-k room, one side of the cabinet where cleaning supplies were located could not lock, the locking mechanism was broken.
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children?s allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.
In each classroom there was an allergy list in their binders. The list had not been updated with new enrolled children since February 2025.
Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine.
Staff were observed changing a child?s diaper in the toddler A room. The staff did not wash the child's hands with soap and water.
The diapering surface shall be used only for diapering or cleaning children.
In the toddlers A classroom a child was observed on the changing table drinking a bottle.
There was no menu posted in the center listing meals or snacks.
Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.
Staff #2, who have been actively working at the center for the past 11 days, only had one complete reference verification on file.
