Inspector Notes
An unannounced inspection was initiated and completed on 9/8/2025 in response to a complaint received by the licensing office on 8/29/2025 relating to allegations of ratio. The Department issued a Special Order on 5/27/2025 to place the licensee on probation for a period of 6 months and mandate training for all staff and any staff that supervise children. The inspector reviewed no children?s or staff records. This inspection included tour of the facility, and interviews.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
