Inspector Notes
An unannounced inspection was initiated and completed on 9/10/2025 in response to a complaint received by the licensing office on 9/7/2025 relating to allegations of forbidden action. The Department issued a Special Order on 6/17/2025 to place the licensee on probation for a period of 6 months and mandate training for all staff and any staff that supervise children. The inspector reviewed 2 staff records. This inspection included tour of the facility and interviews.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to administration, staff training and ratio and are documented on the violation notice issued to the program.

A tuberculosis (TB) screening is required every two years from the last screening. Staff #1 was due for an updated screening 8 months ago.

Staff shall complete a minimum of 16 hours of training annually, appropriate to the age of children in care. Two staff did not complete the required 16 hours of annual training.

The required ratio for children age 3 years up to school age eligible is one staff per 10 children. On 9/5/2025, Staff #3 was alone supervising 16 children, age 3 years, for approximately 20 minutes.
